This Pathway supports children and young people and their families in Devon (excluding Plymouth) within Specialist Pathways in Positive Behaviour Support, Psychological and Emotional wellbeing, Specialist Sleep Support and Health.
The pathway supports:
- Children and Young people with a diagnosis of Moderate, Severe and Profound Learning Disability.
- Have been identified as having a cognitive impairment which is moderate, severe or profound and which limits educational and functional development
- Specific diagnosis may also include neurodevelopmental and genetic conditions such as Down’s Syndrome, Prader Willis, Cornelia de Lange Syndrome. (The list of neurodevelopmental and genetic conditions potentially supported by this pathway is too extensive to list)

- Clinical Psychologists
- Occupational therapists
- Specialist PBS nurse
- Specialist Sleep Nurse

- Psychiatrists
Specialist Learning Disability – Patient Initiated Follow-Up (PIFU) Referral Form
Following discharge, some young people will be placed on a PIFU plan. If your child has been discharged with a PIFU plan, you can contact the service directly for further support during an agreed time period (e.g. 3, 6 or 12 months). This will be outlined in your discharge information.
PIFU means follow-up is led by you, based on your needs, rather than automatically scheduled. It’s offered when further support may be helpful but not routinely needed.
Please only use the PIFU referral form if this option was offered to you at discharge.
