How we process your referral

When we’ve received your form, we’ll send you an automated email confirmation. Please check your junk email folder if you don’t get the email.

Our clinical specialists will look at your referral and decide what to do next. How soon we contact you will depend on how urgent the problem is

We may phone you or ask you to join a virtual meeting so we can talk more about what you need. Sometimes, we may ask you to come to a clinic for an assessment, and your family might be part of that. 

When a decision is made

If our clinicians then decide that the child or young person needs specialist care we’ll develop a care plan for them. This may include further appointments, therapies, or interventions. Parents/carers and families are often involved in the care plan.  The child or young person will receive care from one or more CFHD Pathways to make sure they get the right help. 

If specialist care is not needed, our resources and self-help tools may be useful. These can include CFHD advice and videos, and local and national support organisations.
