Getting help
If you need Children’s Physiotherapy in the Torbay and South Devon area you’ll have to ask your GP or paediatrician to make a referral for you.
If you’re already getting help from another CFHD health professional, such as an Occupational Therapist or Speech and Language Therapist, they can also refer you.
Physiotherapy Team opt in referral form
Following discharge, if clinically indicated some young people will be given the option to self-refer back into the Physiotherapy Team (opt-in) – you will have been informed of this by your physiotherapist. You will be able to opt in for 3, 6 or 12 months from your last appointment based on the clinical need and your clinician will have let you know how long opt-in is available for you. Please do not use this form if you are not under “self referral”, new referrals need to be made by a healthcare professional e.g. GP.
