The Team provide multi disciplinary assessment and intervention support in line with the Thrive model for pre school age children (pre starting Reception) presenting with significant developmental delay in at least two or more areas. This must include cognition, play and communication.
The delay must be 6 months or more for a child aged up to 18 months old and 12 months delay for a child aged between 18 months and 4 years of age.
Patient Initiated Follow-Up (PIFU) referral forms
Following discharge, some young people will be placed on a PIFU plan. If your child has been discharged with a PIFU plan, you can contact the service directly for further support during an agreed time period (between 6 or 12 months). This will be outlined in your discharge information.
PIFU means follow-up is led by you, based on your needs, rather than automatically scheduled. It’s offered when further support may be helpful but not routinely needed.
