What do you need help with: Physical disability
Physical Disabilities will impact on how the child and young person can move their body. There are many causes and severities of Physical Disabilities.
These can include neurological conditions, such as Cerebral Palsy; neuromuscular conditions, such as muscular dystrophy and rheumatological conditions.
We recognise that every child and young person is an individual and their disability will impact their daily function in different ways.
We focus on building on their strengths and helping them reach their potential at a pace that’s right for them. This is a team effort, done together with the important adults in the child and young person’s life.
The most common causes of physical disability include:
- muscular dystrophies
- spina bifida
- cerebral pals
Making a referral
Please note: Children’s Physiotherapy covers Torbay and South Devon only.
All physio referrals will need to be completed and sent to CFHD by a healthcare professional. To make a referral you will need to download and fill in one or more forms and answer questions about what you need help with. Referrals will give our clinical specialists the essential information they need.
It helps if the referral can include:
- how long the difficulties have lasted
- how they affect day-to-day life
- other relevant experiences
You may need to share information from a doctor, nurse, other healthcare professional, or school SENCO (Special Educational Needs Coordinator).
Sending the form back to us
We’d prefer that you email the form back to us at cfhd.devonspa@nhs.net.
When you email us, please make sure:
- the documents you send are only in Word (.doc/.docx), PDF (.pdf) or JPEG (.jpeg/.jpg) formats
- each document is attached separately and is less than 3MB in size
- you don’t attach multiple single pages
- the email is less than 10MB in total
