What is a Communication Passport? 

A Communication Passport is a simple way to share your story, your needs, and what matters to you, so you don’t have to repeat yourself to every professional you meet. It helps people understand you better and support you in the right way.

If you already have a communication passport in place, CFHD will accept other communication passports.

  • Are telling your story again and again 
  • Want professionals to understand you more quickly 
  • Have specific needs, preferences, or experiences that are important to share 
  • Want more control over how your information is shared 

The passport helps make sure your voice is heard. 

  • Young people (including 16+) 
  • Children (with support from parents/carers) 
  • Families and carers 

You can complete it on your own or with someone you trust. 

You can fill in as much or as little as you want. 

There’s no “right way” to complete it, it’s about what matters to you. 

You might include: 

  • What’s important to you 
  • Your communication preferences 
  • Things that help you feel safe and supported 
  • Your experiences (if you want to share them) 

Take your time, you can come back and update it whenever you like.

  • Who you share it with 
  • When you use it 

You can change it at any time. 

How do I use it? 

You can show your passport when you: 

  • Attend appointments 
  • Meet new professionals 
  • Start a new service 

You can say: “I have a Communication Passport. This explains my situation, so I don’t have to repeat everything.” 

You can share a printed copy or a digital version. 

The passport can help: 

  • Reduce the need to repeat your story 
  • Help professionals understand you more quickly 
  • Improve how different services work together 
  • Make your experience feel more joined up and supportive
