Children’s Community Nursing Service: Palliative care

Our Children’s Community Nursing team is here to provide compassionate, high-quality care for children and young people living with life-limiting or life-threatening conditions.

We place children, young people, and their families at the heart of everything we do—supporting you to make informed choices and working closely with other healthcare professionals to ensure care is well-coordinated and delivered in the place that feels right for you.

From the moment of diagnosis, our team can provide support throughout your child’s life, including end-of-life care and bereavement support for families.

Care and support can be given at home, in school, or in other community settings. For children who need end-of-life care at home, we also offer advice and nursing support outside of normal working hours.
