Referral Criteria – Speech and Language Therapy

Speech and Language Therapy (SLT) provide a range of services including universal, prevention, training, assessments and therapeutic interventions/programmes for children and young people (from birth to 18 years) with language and/or speech difficulties, disorders in fluency/voice and with dysphagia.

We work in short focused episodes of care with functional goals which have been discussed and agreed with parents and/or setting. Assessment and interventions follow clear evidence based care pathways.

Conditions in scope

Targeted SLT

Requests will be considered for children and young people presenting with speech and language difficulties. All requests are triaged with consideration given to the impact the speech and/or language difficulty is having. This includes:

Pre-school:

  • Impact on behaviour, such as frustration, hitting out, withdrawal
  • Lacking in means to communicate such as signing, gesture, word
  • High level of concern by parents, pre-school setting

School age:

  • Difficulty in making and maintaining friendships
  • Difficulty processing information and instructions
  • Impact on behaviour, such as frustration, withdrawal
  • Difficulty expressing needs, wants, feelings and ideas
  • High level of concern by parents and/or school

Specialist SLT

Specialist SLT services accept referrals for children or young people with:

  • Moderate to profound sensory-neural deafness
  • Cochlear implant
  • Mechanical feeding and swallowing difficulties
  • Maxillo-facial disorders including cleft palate

The SLT service uses Care Aims principles to support its clinical decision making at triage, assessment and through any intervention offered. This approach identifies the impact and clinical risk to the child or young person and supports the decision as to whether SLT is the appropriate service at that time. Please ensure that you indicate within the Request for Service form the impact the difficulty is having/perceived to be having on the child or young person.

Please review the list below and ensure that you have the appropriate documentation available to submit with your request – items in bold are required (by age appropriateness):

  • Early Years toolkit – summary/report of completed activities with outcomes
  • ENT (consultant) report – requests for voice difficulties only
  • Hearing test – most recent report – requests for speech difficulties only
  • Let’s Talk More screening tool for children aged between 2 yrs and 2 yrs 11 months – results of screening and score
  • School Years toolkit – summary/report of completed activities with outcomes
  • Speech Link/Language Link assessments – results and evidence of outcomes

Guidance and useful information

Please review the list below and ensure that you have the appropriate documentation available to submit with your request – items in bold are required:

  • Completed Request for Services form (to be completed by Requester)
  • Evidence that strategies from website (such as toolkits) have been tried, with evaluation and outcomes
  • ENT (consultant) report – requests for voice difficulties only
  • Hearing test – most recent report – requests for speech difficulties only
  • Let’s Talk More screening tool for children aged between 2 yrs. and 2 yrs. 11 months – results of screening and re-screen if appropriate
  • Speech Link/Language Link assessments – results and evidence of outcomes
